Social Worker
ROLE DEFINITION
Operating within their full scope of practice, the Social Worker will work co-operatively with all members of the health care team in the promotion of health and prevention of disease.
It is expected that the Social Worker will have both counseling and psychiatric experience (including familiarity with common psychiatric disorders and a general understanding of commonly used medications and their side-effects and interactions) in order to provide comprehensive mental health care from a population health perspective in accordance with evidence based clinical guidelines and best practices.
The Social Worker will provide comprehensive and coordinated mental health services to rostered patients. This will include therapeutic assessment, formulations and appropriate treatment. The spectrum of care will range from early prevention to offering limited time counseling. Interventions will focus on self esteem, dysfunctional communication patterns, and maladaptive coping strategies. The Social Worker will also act as a Health Promoter alongside the inter-professional team. (S)he will participate in an ongoing assessment of patient needs and will develop appropriate programming to meet the identified needs. The Social Worker will establish and maintain ongoing liaison with agencies and community resources that are relevant to the health promotion and disease prevention programs and services.
LIST THE SPECIFIC DUTIES OR RESPONSIBILITIES
- Conduct psychosocial assessments, formulations and detailed intervention plans on those patients referred for service. Including the geriatric, adolescent and paediatric population
- Develop short-term therapeutic relationships to foster coping and adaptation to health promoting strategies or chronic conditions
- Provide a range of therapeutic interventions including individual, couple, family and group counseling and psychotherapy on a short-term basis
- Management of common mental health problems
- Maintain accurate and concise confidential EMR documentation pertaining to patient history and encounter data
- Provide oral and written feedback to necessary inter professional staff
- Contribute to clinical teaching and training of residents and interprofessional team members.
- Establish and maintain ongoing liaisons with the agencies and community resources that are relevant and necessary to the integration of programs and services, and high-quality patient care
QUALIFICATIONS
Professional/Education/Experience Required:

Formal, Minimum Education Required
- Master of Social Work Degree from accredited Faculty of Social Work. This degree must be accepted by the Ontario Association of Professional Social Workers
- Must be eligible for membership in the Ontario Association of Professional Social Workers and the Ontario College of Social Workers.
Professional/Education/Experience Preferred:
- Experience working with families from diverse communities and backgrounds
- Experience in community health and multi-disciplinary team model
- Knowledge and awareness of issues related to homelessness, immigration, mental health, poverty and discrimination
- Ability to work in an inter-professional team environment
- Ability to prioritize, manage time effectively and be flexible in a very active work environment
- Effective communication/interpersonal skills
- Strong clinical skills
- Excellent oral and written skills
EDUCATION/STAFF DEVELOPMENT
- Determine the need for and implement health promotion and disease prevention counseling
- Provide teaching to Family Medicine Residents, Medical and Nursing students and interdisciplinary team members where it would help in their practice
- Participate in ongoing research related to client groups of the FHT
