Registered Dietitian
The Credit Valley Family Health Team (CVFHT) is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging multi-disciplinary primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities.
The Inter-Professional Care Team (IPCT) is a unique model of care where family physicians, social and community service providers work together with Inter-Professional healthcare Providers (IHPs) in an integrated manner to provide comprehensive healthcare. We deliver care to all patients in need, with a special focus on individuals with complex needs, marginalized, low income individuals, families and unattached residents in the community. As a member of the IPCT, you will work alongside Nurse Practitioners, Registered Nurses, Pharmacists, Physiotherapists and Registered Social Workers to deliver care.
Your Qualifications:
- Baccalaureate of Science in Nutrition or equivalent
- Registered Dietitian in good standing with the College of Dietitians of Ontario
- A minimum of two years (2) of Diabetic and Nutrition employment experience preferably in a community-based health care
- Experience working effectively within a busy inter-professional clinical environment
- Excellent oral and written communication skills
Your skills:
- Previous experience in chronic disease management; pediatric nutritional issues; and digestive disease disorders; seniors; adults and youth
- Experience working with complex marginalized, diverse communities
- Comfortable to provide virtual care counselling support to patients and families - if needed
- Ability to work in an inter-professional team environment and respond to nutritional concerns / questions
- Demonstrate ability to facilitate group educational sessions using the principles of adult education
- Ability to effectively prioritize work required
- Commitment to ongoing professional development
- Application of evidence - based practice
- Computer literacy and familiar using digital health intelligence for e-referrals
- CDE designation (optional)
- Bilingualism is considered an asset
Main responsibilities:
- Responsible for assessment, development of a treatment plan as well as implementation of the treatment in collaboration with the client's stated goals within scope of the clinical discipline
- Assess client needs and link to appropriate community resources
- Maintain accurate, concise and confidential EMR documentation where applicable, pertaining to patient history and encounter data
- Complete necessary FHT and MOHLTC program documentation
- Knowledge and delivery of health promotion and program planning is an asset
- Working in a multidisciplinary professional setting to provide care to patients and access to full spectrum of health conditions with focus on Seniors, Adults, Youths, Pediatrics and Mental Health and Addiction.
- Other responsibilities as assigned by the IPCT Manager, the physician leads and Execute Director
Job Types: Part-time, Fixed term contract
Contract length: 6 months - with the possibility for extension
